On which days should I complete the online dietary questionnaire?

We would like you to complete the questionnaire on two separate days, which the healthcare professional will write on the front of the online dietary questionnaire leaflet.  If you provide us with a mobile number or email address we will send you reminders on the day the questionnaire should be completed.  If you are not sure, please contact us on Freephone 0800 652 4574 or ncds@natcen.ac.uk and we can tell you which days.
